Not sure about the X-Men, but you know, I think the only person in any comic series who ever died for good, EVER... Was Gwen Stacy from Spiderman.
The Punisher, on the other hand, has never died. He's too badass to die. You know, there's actually a comic called "The Punisher Kills the Marvel Universe" where the Punisher goes around and... well, he does just that.

Straight-up, ain't comin' back X-Men deaths:
Changling - although he would be later re-invented as Morph... but technically not the same character
Banshee - hit straight on by an airplane
Joseph - a clone of Magneto, he sacrificed himself while fighting the real Magneto
Revanche - assisted suicide in the face of having the Legacy Virus
Thunderbird - blew up with an ariplane
Kuan-Yin Xorn - as stated by the flash animation

lol, wow.. Is whoever writes the comic too afraid of disappointing fans for too long or something? That one reason that comic book deaths happen. Another is for the cartoonists to bring attention back to their work (ie - Superman's death.... GOOD GOD did I hate Superman's death).

But I must say... watching Superman getting his ass handed to him for a comic and a half was amazing. And when the big S finally died, Lois Lane was bawling.
I still remember the sweet taste of her sorrow.

First time around as in before Doomsday?
I know that a villian named Gog killed Superman then went back a day in time and killed him again and repeated this each day until the day that the child of Superman and Wonder Woman was going to be born. This was done to explain "hypertime" which was going to explain the all the continuity errors of the DC universe. A concept that has since been disavowed DC comics.
Another reason as to why DC comics suck.
